We are seeking a full-time Nurse to join our Patient-Centered Medical Home team. This role provides clinical support to providers, patients, and families to ensure efficient workflow and high-quality patient care. Evening and weekend shifts are required on a rotating basis.

Key Responsibilities:
  •  Administer vaccines, injections (including Rocephin), and perform venipuncture 
  •  Conduct telephone triage and apply appropriate clinical protocols 
  •  Collect, process, and manage lab specimens, including in-house testing and quality controls 
  •  Maintain accurate and timely patient documentation and communication records 
  •  Communicate test results and follow-up instructions per provider orders 
  •  Initiate referrals and coordinate care, including escalation to emergency services when appropriate 
  •  Ensure compliance with safety, infection control, and patient confidentiality standards 
  •  Support providers and collaborate with the care team to ensure quality patient care 

Qualifications & Skills:
  •  Strong clinical knowledge, including emergency protocols and pediatric care 
  •  Excellent communication, customer service, and conflict resolution skills 
  •  Highly organized with strong attention to detail 
  •  Ability to multitask and adapt in a fast-paced environment 
  •  Proficient in computer systems, documentation, and phone communication 
  •  Professional, team-oriented, and able to work both independently and collaboratively 
  •  Committed to maintaining patient privacy and high standards of care

QUALIFICATIONS:
  • Education: Minimum high school diploma or equivalent.
  • Certification: Licensed Practical Nurse or Registered Nurse Certification
  • Experience: 1-2 years experience in the medical field preferred
